Billy DeMoss Shakes Up Appleton Wisconsin at Petty Michel Seminar

Dr. Billy DeMoss gave a heated and edgy presentation for 3 hours to chiropractors and their staff in Appleton, Wisconsin, Friday night, Aug. 19th. Full of anecdotes from his own practice, along with statistics, slides and videos, “Billy D” reminded everyone what chiropractic and natural health care is all about.

He talked about how providing chiropractic care is actually fun and how you should have fun in your office. He stated that patients want to come to a place that is enjoyable, where staff and doctors are having a good time in a positive atmosphere.

But he also explained that it takes a great deal of work to have a successful office. He recounted the days that he had to do manual odd jobs just to make it through school. He works hard at his office but keeps it fun.  “If you want a more successful and profitable practice you are going to have to work for it.”

“The days of the $200 dollar office visit are fading,” he said.  Workers Compensation reimbursement in California has been cut back and some money motivated chiropractors have abused the system. But money is not the goal, and never has been, he said.  Serving people will always be rewarded financially, but that is not our primary objective.

He emphasized that our main goal should be to teach and adjust. Some doctors complain about doing screenings or lectures, but Billy asks the audience: who do we do it for?  Are we worried about inconviencing ourselves or do we want to help people get healthier?

He discussed issues such as vaccinations, diet, United States health statistics relative to other countries, and said that the chiropractors were the ones best poised to take a leadership role in the health care of the U.S.

After 3 hours of Billy’s high energy, the mesmerized and energized crowd gave him two standing ovations.

One member of the audience who owns a small farm and who was not a doctor or staff member, said he wished there were more people speaking out like Dr. Billy DeMoss. He said it was one of the best presentations he had ever seen.

The next morning,  Phyllis Frase spoke to staff and doctors about how to be a “Rock Star” C.A.  Phyllis also gave a dynamic talk and will be speaking at the Cal Jam in California, February 24-26.

Timely Filing of Insurance Claims

We recently received an inquiry regarding the timely filing of insurance claims and where the responsibility of a denied claim lies.  As many offices may be faced with this similar situation, we would like to share this helpful information with you.  Names have been changed to protect the innocent.

Situation/Question presented:
Situation is we have a patient who was seen two times in our office for a PI $395.  Our policy is to bill the auto med pay portion of the patient’s auto policy. So we billed their auto insurance.  Two months later his wife asked us to bill their health insurance,  so we did .  Months later the bill still was not paid by either insurance  so we resubmitted to the health and it was denied for timely filing saying they did not receive our initial billing.  So before billing the patient I called the auto insurance to see if we could seek payment from them. I was at that point informed they settled with the patient like 8 months ago. they would not tell me the amount but did confirm they had all our claims on file.  I then put together a letter to the patient along with a bill for the full amount.  The patient recently called me disputing this bill saying that in the state of WI  there is a law about collateral source of which states she is allowed to collect from both ins companies and that because of that she is not responsible for the bill.  She said their health insurance  told her because we did not send the bill in time that she is not responsible for it.  She basically is refusing to pay and is threatening us hearing from her attorney if we pursue her.

The way I see it is even if there is a collateral source it doesn’t have anything to do with us. She tried to collect from both parties and it didn’t work out so now she is mad she isn’t profiting from her injuries if she ends up paying us.  As far as her health telling her she isn’t resp. because we didn’t send it in on time. I don’t think they can say that. Isn’t she still responsible no matter what? I have two different lien forms signed by the patient.  I don’t believe we are doing anything that an attorney would even bat an eye at.  We are talking about two visits totaling $395, is it worth it???
I’m just confused on how to move forward with this and wanted to get a second opinion  on this matter before I do so.

You are correct. The patient is personally responsible for the bill. Her auto medpay was primary and should have been billed first (which you did). You should have received a denial from them (not sure why you did not, but you can request one) and that should have been forwarded to the health insurance  for any “timely filing” concern.

I have attached an article on disputing the health insurance timely filing claim.  Article
Her attorney (if he or she is ethical) will tell her the same thing, that she is responsible for the bill for services rendered.
So you can do a few things here: rebill to health insurance (disputing the timely filing denial) and balance bill the patient for her portion (deduct/copay/coinsurance) or just send the patient to collections.

I would try the nice approach first and rebill to the health insurance . Make sure you document your collection efforts because I’m sure she will dispute the collection agency.

Patient Compliance in a Chiropractic Office

The success of your chiropractic practice is directly related to the level of patient control you and your staff have with your patients.

No matter if you are the best healer this side 1895, or if your office is perfectly modern and your team is the friendliest in town, you will fail in business if you are weak on control.

Once your patients agree to their treatment plan, can you get them to comply? If you can’t, they won’t really get or stay healthy and neither will your practice.

You have to be friendly and have good communication, of course. But you also have to be a coach. A good coach, if you have had one, is just a bit and demanding. They are this way because they care. They care about winning and they care about their players.

If you care about your patients “winning” back their health, and staying healthy, then you have to be willing to be assertive about them sticking to their schedule of care.

And if you aren’t being insistent with your patients, we know that the pharmaceutical companies certainly are with their constant commercials and medical prescribers.

Holidays are a time when chiropractic patients can find many reasons not to keep to their schedules. To help you and them stick to their health program, you can pre-schedule them for the entire month of December.

When talking to your patients about their schedules, be understanding and empathetic. Hear their story. Then, be bold about your interest and concern that they maintain their care schedule through the holidays – and get them scheduled until next year.

And it is OK to use bribes.. Spicy warm apple cider gives off a very cozy smell and tastes great.. and is healthy, and free screenings/exams for patient’s holiday guests.

Internet Marketing for Chiropractic Offices

What a great teleseminar we just completed about Internet Marketing.

I wanted to provide a short summary and a few follow ups for those who joined us as well as for those who weren’t able to.

Face To Face Networking – now more valued than ever. As we live increasingly in a virtual world of “friends” and emails and ordering products and services from our computers, meeting and connecting in person with real live people is rarer and therefore more valued. Get out of the box and meet people!

Virtual Networking – We discussed Facebook and creating a fan page. Here is a link provided by Dr. Jamie Phillips on how to do this. (link) There are many ways to add content, or information, to your Facebook page including automatic feeds.  Dr. Phillips gives one example, using Google Alerts,  listed below.  But automated content or ads about upcoming events do not take the place of “newsy” and interesting articles about you and your office. A fast note about a patient success, something new about the office, a favorite recipe of one of your patients,  a notice from Sister Hilda (a patient) who is inviting one and all to the fish fry at Saint Paul’s this Friday – these are topics your patients are interested in.  Keep it local.

The Internet Is About Search. It replaces the phone book, the dictionary, and the encyclopedia. There are many people who are in pain and sick of drugs and corporate medicine and surgery. They are looking for you. Dr. Phillips went over how to best get these people to find you with local search.

Web Site. We also discussed some simple ideas for your web site and what to do if you already have one.

Other Topics Included: The importance of Social Proof, Reputation Management, use of the new bar code on you printed materials and signs, and the coming necessity of mobile web sites were other subjects we spent some time on.

Chiropractic Internet Marketing Assistant. You need one of these!   Delegate a team member to put 2 hours in per week, or more, dedicated to working on your internet presence.

We did record this teleseminar and it will be available at no charge for our active clients on our Members site in about a week. Non members who listened to the talk will receive a CD within the next several weeks.  The CD can be purchased for $65 – all proceeds will be donated to Oklahaven Children’s Chiropractic Center.

Fall is in the air….. and it’s time to get that fall marketing calendar filled with activities.  To get the ball rolling, we have put together a few topics and suggestions to make your events fun and effective.

Internet Marketing That Works

A web site is nice, but can it really generate new patients for you?

The answer is yes! And over the years, depending on the doctor’s internet marketing efforts, we have seen more new patients calling the office because of something they read on the Internet.

You probably have a web site. It could be a templated one you subscribe to or your own customized site.

But having a site is the easy part – it’s what you do with it that counts.  It is not like a phone book ad that you redesign every 12 months and then get conned into buying a larger ad by the phone book rep.

We have been studying internet marketing for many years now. It is a science and an art unto itself, with new approaches and technologies evolving monthly.  But there is also an awful lot of hype. Every company has a different pitch about how their web site is better. There are new gurus being born every week. Then, at a family barbeque, your 14 year old geeky nephew says he can throw up a web site for you over his lunch as part of a school project.  So, what do you do?

Because of all these things, your web site may just sit there.  Maybe it catches something now and then like a fishing lure, but more likely, it is mostly ignored and has floated to the river’s edge doing nothing.

We have a solution to this.

We will be sending more info about this soon. But the short version is that someone in your office should be able to work on your Internet marketing every week. Like any marketing, it only works if it is done.

We will be introducing the Chiropractic Internet Marketing Assistant Checklist and Support Program soon. Believe me – the Basic Level will be so easy even a Caveman, or Cavewoman, can do it.  (No offense intended!)

The internet is becoming more of a marketing medium for your office – and can be effectively used for generating new patients and keeping the ones you have.

The bottom line? 1-3 new patients for a few hours spent each month working on Internet marketing. Not a bad return.

We just finished our 10 classes in 1 day seminar in Minneapolis – our “10 in 1.”  From what I heard and saw, everyone had a great time. I know we did. And for my money, I think we had some of the best professionals in the country giving presentations – as well as attending!

Meeting Family Deductibles

This article was sent in to us by one of our clients. We felt it was a great suggestion and thought it might help your office when faced with the same situation.

“I just wanted to pass something on that may benefit your other offices.  If we have families that we treat where only two or three of them need to meet their deductibles, we choose the family members that are incurring the most charges and send those in right away.  The other family members we hold the charges until the family deductible is met.

“For instance, we have a family of about 8 people that we are treating that have to meet two deductibles to make their family deductible.  We are only sending CMS-1500 forms on two of those patients until the deductible is met, then we will submit all the bills from the other family members so that we don’t have bits and pieces being eaten out of everyone’s claims unnecessarily”

Filing in this manner, simplifies the collection of the patients out-of-pocket at the front desk, reduces the number of claims affected by the deductibles and overall increases the efficiency of the accounting for these patients.

What’s Been Working: 7 Characteristics of Successful Chiropractic Offices

We recently reviewed what has been working marketing-wise with many of the more successful chiropractic offices with whom we work.  Many chiropractors are doing their best ever, even some who have been in practice for years.   One called up last week and said they had over 30 new patients that week.

What did we find? There seems to be a few key elements in common.  Here are 7 of them:

1. External Marketing Procedures. Many offices have had very successful external events over the last several months.  These are often scheduled a year in advance, and include talks, screenings, and networking. But external marketing also includes advertising, such as a dynamic web site, email, and even some radio, television, and print.

New offices, in particular,  need to concentrate on external marketing. But established offices also benefit because they not only help generate new patients, but also reactivate former patients and bolster the confidence of your active patients when they see you outside of your office.  We have lots of materials, posters, ads, procedural manuals on these external procedures. Many are free on our web sites (here and at pmaworks.com). You can also purchase our Marketing Manager System with the MMS Marketing Toolkit which has hundreds of practical marketing materials for your use.)

2. Internal Events. Most offices that are doing well have had some kind of internal event. Successful internal events are fun. They can simply be something silly like a costume day (Halloween Costume Awards), 80’s Day with mullets, or a  “Fruity Friday” with fruit on Fridays (Yea Team Munson!).  They can also focus on referrals: “Bring a Buddy Day”, services for donations, coupons, etc.  A very effective program is educational classes, such as the standard spinal care class and monthly advanced classes on different conditions and topics. (These procedures are also covered on our web sites and on our MMS computer program.)

3. Patient Education. Offices that have been doing well work hard at “telling the chiropractic story.” True health sets us apart from the medical world, which concentrates on crisis care and disease care. The more the patients understand this difference, and seek it, the more they stay, pay, and refer. Nothing beats an educated patient. A patient that understands the importance of spinal hygiene and general organic health will be more than a patient but a fellow team mate in your quest to help others regain and maintain their own health. Along with this understanding is the need for you and your team to also understand the opposing forces to healing from the inside, which include personal laziness and irresponsibility, Big Pharma, Big Food, and toxicity in every day materials.

4. Clinical Focus and Certainty.
Successful offices have doctors that continue to be engaged in the craft, science, and philosophy of their profession.  I have seen offices with full appointment books simply because the chiropractor was a zealot about his skills and outcomes and was certain that he could nearly raise the dead.   One definition of professionalism could be the act of providing the utmost in excellent service long after the excitement and newness for the subject has evaporated. This requires discipline to constantly renew your eagerness for your skills.

5. Excellent Team Support. The busy offices were a team where everyone helped the patients and doctor quickly and cheerfully. To do this you have to have the right people in the right roles doing the right procedures.  Sometimes the office is disorganized, or the front desk staff member wants to be an airline stewardess (attendant) and the billing coordinator wants to work in a hospital. Sometimes there simply needs to be another staff member helping, or the procedures change too often for no reason.  These can stop new patients.

Simply put, someone has to do the marketing. Your team should want to sell heath. If the office is sufficiently organized and motivated, new patients seem to walk in as if being summoned telepathically.

6. Clinic Atmosphere. The quality of the atmosphere of an office is usually taken for granted. And given the fact that other practice building elements are in place, it may not be an acute problem. But there is no question that in the long term, the “vibes” of an office have far reaching effects.

Corporations understand this and make attempts at creating a great atmosphere. Starbucks is an example. But small town or corner stores often do this better, where the service and care is down right…neighborly. People want to go to a place where everyone knows their name.

7. Executive Skills.
Except for the clinical component, most of the elements mentioned above rests upon the skills of the doctor as a business executive, a role for which you did not necessarily sign up for when you paid your tuition at chiropractic college.

Every successful business has a C.E.O.  who inspires the team, ensures successful polices and procedures are constantly applied and improved, and plots the long term growth of the enterprise.  This is actually THE missing role in most chiropractic offices, by the way.  This is why most doctor’s stay chained to their jobs and live week to week, working to pay their overhead and keep the doors open.

To convert a job to a business requires someone to move outside of the practice and start to work on the practice, not just in it.

This is such an important skill that next year we will be starting a new series on The Chiropractor as the C.E.O.

Minneapolis Workshop, April, 2009

What a workshop and boot camp! With 4 separate breakout sessions, everyone was busy learning, training, and getting inspired.

We learned a lot too, and for those of you who attended – thank you!  We had at least as much fun as you did.

We have had many glowing responses since we ended the seminar, including this one, the day after the seminar. One doctor called in and said:

“I don’t know what you guys did, but we are on fire! Heather loved Phyllis and sold THREE PREPAYS this morning!”

The exuberant doctor said that two new patients came in with a guest.  All three became new patients, prepaid, and left with full appointments. “I ended up with an additional 96 visits scheduled”, the doctors said.

New England Chiropractors Are Amazing!

Does New England have the most gung-ho and happy chiropractors in the country?

We just returned from an amazing week in the Boston area where we had the privilege of spending time with local chiropractors and their teams.

Wow! We doubt that we have ever seen a group that was more centered and positive about chiropractic and the services they were providing to their patients. And after spending a day in one of the offices near Boston, we are also convinced that there are no patients who are more satisfied with their services.

On Thursday, November 6, we hosted our 3-Goals seminar to over 50 doctors and chiropractic team members. Presentations were provided by Dave Michel, Phyllis Frase, and myself, with our special guest who gave a phenomenal lecture, Dr. Peter Kevorkian.

Dr. Peter is also a faculty speaker with the International Chiropractic Pediatric Association.

The next day, Friday, Dave and I were interviewed in a television studio for a cable television lecture series produced by Dr. Jean-Marc Slak. Saturday was topped off by a presentation to the Massachusetts Alliance for Chiropractic Philosophy.

What Do You Think About When the Phone Rings?

You are sitting there, trying to finish your notes. You hear your phone ring. You are a bit behind. Maybe slightly irritated by an arbitrary denial of an insurance company and you haven’t yet planned out tonight’s evening with the family, spouse, or friend.The phone rings again.

What are your thoughts? What are your feelings?

Do you kinda wish it wouldn’t ring? Is it a bit of an interruption? Do-you-just-want-to-answer-the –dang-phone-to-stop-the-ringing-so-you-can-get-back-to-your-work?

Essentially, your thought is “Stop.”   It is: “Don’t call me.”  “Phone, don’t ring, don’t interrupt me!”   I am sure this has happened to you – even if ever so slightly or subconsciously.

Now, imagine if your front desk has these thoughts when the phone rings?

To some degree, even the most devoted and hard working staff can reactively feel put upon by phone calls. Or, in fact, by walk-ins. Or, in fact, by any patient encounter. I have seen this happen on the front desk when the doctor was busy with patients.  But remember, this can happen with even the most ethical team member, including you!  I have seen doctors do this too. (Extreme examples: “Oh, only two people for the spinal care class – cancel it.” “Oh, just Jim is coming in, reschedule him. I am going to leave early. It is MY office and I can do what I want.”)

Our thoughts can and do determine our behavior and affect how we treat others. Our environment actually mirrors our thoughts.

You may have heard or read about this scientist in Japan. Fascinating experiments. The fellow’s name is Dr. Masaru Emoto and was also in the movie, “What the Bleep Do We Know?”

According to the movie, Dr. Masaru Emoto claims that crystals formed in frozen water reveal changes when specific, concentrated thoughts are directed toward them. He says that he found that water from clear springs and water that has been exposed to loving words shows brilliant, complex, and colorful snowflake patterns. In contrast, polluted water, or water exposed to negative thoughts, forms incomplete, asymmetrical patterns with dull colors.

So, if our thoughts can affect the formation of water crystals, they probably can have significant effect on our office.

Going back to your front desk, realize that the staff on the front desk have tremendous control over the office, nearly as much as the doctor does. The front desk can be a magnet for your patients, and attract, or repel them.

When the phone rings, you want your front desk, and all staff thinking “YES.”  “Call me. Phone –  ring now.”  “I can hardly wait to talk to this person and see how they are. I am interested in them and how they are doing. I want to help get them come in for care — and their family too.” “They must be really really cool and nice if they are calling us.” “I WANT to know more about them.” “I am grateful for their call and appreciate the effort they made in calling us.”

These are good thoughts.  These are positive thoughts that can help bring in more patients.

You can practice this with your staff at a team meeting.

ACTION STEP. As an example, someone acts as the prospective patient calling. The person acting as the front desk assistant should answer the phone with the attitude of really not wanting to talk to the person. Act it up. This can be funny. Try it a few times.

Then, do the same rehearsal with the front desk assistant positively anticipating the phone call, wanting the phone to ring, and then eagerly answering and talking to the prospective patient.

Keep it the role playing brief. You can and should do it again. It should be fun and act as a reminder to one and all that we should want to meet new people, talk to existing patients, and look forward to phone calls. We can always dismiss the occasional telemarketer or wrong number.

This can also be rehearsed in other types of patient encounters, from taking the new patient back to the exam room, sitting down to do a financial consultation, or checking a patient out and collecting their payments.

ACTION STEP. And speaking of how our environment does mirror us, here is a little trick you can use.  Get a real mirror, about 4 inches by 4 inches, and put it at the front desk counter so that the team member can see it. You can write something on it like: “Are My Teeth Showing?” “Am I Smiling?” And staff, you can also put one of these in your doctor’s office on any day that he is feeling grumpy. It applies to us all.

So, the next time the phone rings, smile. Be interested and curious in who is taking the time to call.  Make your thoughts happy so they help create an office that is brilliant, colorful, and full of happy patients.

How Do You Start Your Day?

How do you start your day?

The fact is, the way you start your day will determine, largely, how you end your day.

Start it right and you’ll end it right.

Start late, flustered, groggy, moody, cranky, and your day will be longer, harder, and less productive.

Time and time again, when we work with doctors and their teams on starting their day off correctly, we notice that the patients become happier, the appointment book fills up,  and the doctor feels more in control of her patients.  Just last month, as a matter of fact, one doctor implemented this procedure and hit her highest new patients – ever.

Another doctor gets to the office before his staff and associate doctors and reviews the day’s schedule.  Before the patients come in, he reviews each patient, as needed, and makes sure all patients are set up for the right procedures that day. In spite of the fact that he has several clinics, and a handful of associate partners, it is no wonder he still averages over 1200 visits per month. So can you.

You can start your day any dang way you want, the choice is yours. Actually, you have two choices as diagrammed below.

Some offices call this their morning “huddle”, or “case management” meeting. Whatever you call it, here are some successful procedures to start your day:

1.    Discipline. Make sure that it starts on time. Only have it 2-3 times per week to start with. Keep it simple, FAST, and fun. This will be the biggest hurdle. But, just schedule it and make it happen.

2.    Humor. A staff member should start off with a good joke. For example, the front desk has the first Monday, the doctor on Wednesday, the Accounts CA has to bring a joke for Friday, and so on. It goes on a rotational basis. If this is hard to do, then that is all the more reason to do bring a joke. If you are bad at jokes, again, all the more reason to bring a joke. 🙂  Need a joke? We have a few over here.

3.    Patients. Then, go over each patient that is coming in that day and decide, as needed, what extra should be done with him or her.  For example, does Patient A need a progress exam, need to be scheduled for the care class, another financial consultation, or be seen to schedule her family for no charge “check-ups?”  If your office is seeing over 800 patients that day, probably best to just check over some of the patients — no need to cover each one. Just see if any of the doctors or staff have any input on anyone.[We have seen an 800 visit day. Lots of fun!]

4.    Office. Then, review how the office is progressing towards its goals for the month.

5.    Goals/Visualization. Then, set realistic daily goals, and look at and even visualize not only achieving them, but achieving the weekly and monthly goals as well.

6.    Miscellaneous. No more than two quick minutes for a miscellaneous announcement, if needed.

7.    Motivation. Finally, the clinic director should end off with a patient success story, or something inspirational and motivational. A quote, a story, etc.  We have lots of quotes in the Marketing Manger System computer program. You can also take one Chiropractic Principle each day, discuss it with the staff, and work out ways to apply it that day.

The whole meeting should be done BEFORE patients are seen, and last no more than 15 minutes.  Use it in your group, and use it on your self, and watch your practice grow.

We just finished our Minneapolis seminar and WOW! Not trying to exaggerate here, but it was great.


Highlighting our the seminar was Dr. Peter Kevorkian who gave a rousing presentation. Dr. Peter’s animated talk interwove science, chiropractic history, and practical patient education procedures that both inspired and informed.

Like many of you, I have been to scores, if not hundreds of chiropractic talks (not counting listening to Dave go on and on about insurance companies on our long driving road trips). Dr. Peter was masterful at integrating philosophy and science with the practical nuts and bolts our clients could use to develop their practices. One of the best presentations I have heard.

And like a true leader, he practices what he preaches, seeing well over 3,000 visits per month with his wife, in three days per week. He actually is achieving all three goals and represents our 3 Goals Program well.

Phyllis Frase gave specific tips for staff team members and doctors in her presentation that was energetic and passionate. Dave Michel and I also gave talks that provided sound business and marketing advice.

One doctor almost complained to us that the day after the seminar his staff were so on fire and motivated he could hardly recognize them.

One of the most glowing reports stated that the seminar was a “Good balance of hard facts and philosophy.”

What a Seminar! Our second 3 Goals seminar which was held in Milwaukee was SO much fun that if we knew it was going to be like this, we would have been doing seminars years ago.

With over 60 people attending,with veteran doctors and staff, and rookies as well, the place was rockin’.

Party at Chiroville

Many mall stores, such as the GAP, Banana Republic, Abercrombie & Fitch, have as part of the office policy to create a party atmosphere in their stores. The idea is that potential shoppers passing through the mall will see happy faces in the stores with the music playing (sometimes extra loud) and feel like they want to join in the party.

The same concept applies to your office. It should always be friendly and inviting. And on occasion, it’s OK to go overboard.

Early this week, one of the chiropractic office’s I visited had a pleasant surprise. I made it through the ice, snow, and sub-freezing temperatures to the front door, took off my heavy coat, and walked into the reception area.

The staff were dressed in summer clothes. Marimba island music was playing. There were palm trees and a fishing net over the front desk. The staff were smiling and one of them asked me if I wanted a fruit smoothie, being told that they were not serving rum this morning.

The sign over the blender and freshly cut fruit said: “Welcome to Chiroville”, alluding to Jimmy Buffet’s Margaritaville. While enjoying my freshly made smoothie, I watched other patients come in from the cold and saw how pleased they were with the friendly change of pace of this special promotion.

The staff seemed to have as much fun with it as the patients. And, as did the doctor!

As a side note, the practice is having its best February ever for new patients.

The moral of the story? If every day was a party at your office, wouldn’t you want be there? Wouldn’t you want to invite all of your friends there too?
